Dr. Thomas Souza, DACBSP is the current
Dean of Academic Affairs at Palmer College of Chiropractic West and is the
author of Differential Diagnosis and Management for the
Chiropractor: Protocols and Algorithms, 3nd Edition and Sports Injuries of the Shoulder:
Conservative Management. Dr. Souza has lectured nationally
and internationally on many different topics ranging from “Chiropractic
Treatment of Neck and Low Back Pain” to “Differential Diagnosis and Management
of the Upper Extremity”. Dr. Souza was also a columnist for Dynamic
Chiropractic and served as a consultant for the Mercy Center
Guidelines
Dr. John Carr is currently the Medical Director and Chairman
of the Utilization Review Department for Chiropractic Associates, Ltd. of South
Dakota (CASD) and Chiropractic Associates of Minnesota (CAMN). He is
currently a member of the disciplinary and ethical committee of South Dakota
Board of Chiropractic Examiners, as well as a member of the standards of care
committee for the South Dakota Chiropractors Association.
Dr.
Carr is past president of the South Dakota Board of Chiropractic Examiners,
past chairman of the South Dakota Chiropractic Peer Review Committee and was a
special investigator for unprofessional and unethical conduct for the South
Dakota Board of Chiropractic Examiners.
Dr.
Carr attended the University of South Dakota and graduated from National
College of Chiropractic in Lombard, Illinois. He is certified in sports
medicine, rehabilitation and meridian therapy / acupuncture. Dr. Carr has
been in full-time practice at Carr Chiropractic Clinics, PC in Miller, Huron,
Redfield, Faulkton and Kimball, SD since 1980.
Dr.
Carr lectures on Chiropractic Physician Standards of Care, Quality Assurance
Guidelines, Proper Protocol, Documentation, Ethics, and Insurance Relations.
Dr. Wayne H. Carr is
a graduate of National College of Chiropractic in 1987. Dr. Carr is certified
in Acupuncture and Sports Injuries, attained Diplomate status in Rehabilitation,
and is a Corrective Exercise Specialist (CES) and Performance Enhancement
Specialist (PES) through the National Academy of Sports Medicine.
Dr. Carr has lectured on rehabilitation topics and is currently on the
educational team for the CES and PES programs.
Dr.
Carr is part of the DELPHI group for the CCGPP project and has been involved as
a consultant to various insurance companies and management organizations.
Dr. Carr has been instrumental in developing the Best Practices in Chiropractic
Academy. Dr. Carr has been in full-time practice
at Carr Chiropractic Clinics, PC in Miller, Huron, Redfield, Faulkton and
Kimball, SD since 1987.
Dr. David Fisher is
a 1993 cum laude graduate of Palmer College of Chiropractic, Iowa. He
holds certification in Chiropractic Spinal Trauma from the International
Chiropractor's Association and is a Diplomate of the American Chiropractic
Rehabilitation Board. Dr. Fisher has lectured primarily on rehab
protocols. He is in private practice in Duluth, Mn.
Dr. Jeff Gullickson is
a 1991 graduate of National College of Chiropractic. He has been a team chiropractor for 3 high
schools. He has co-authored chapters for
Dr. David Stude’s Spinal Rehabilitation and Dr. Steven Yeomans’ The
Clinical Application for Outcome Assessment; along with research articles
in JMPT. Dr. Gullickson has lectured for
NWHSU Rehabilitation Diplomate program.
In addition, he has given lectures on Chiropractic, ergonomics, and
self-care for Hamlin University, Minnesota Medical Institute, and Minnesota
Radiation Therapists.
Dr.
Gullickson is an emergency care instructor for the National Ski Patrol, and
received his National Appointment in 2003.
He has lectured on balance training for the National Ski Patrol and the
Professional Ski Instructors of America.
He is currently in private practice in Hopkins, Minnesota.
Dr. David Stude is a 1984 graduate from National Health
Sciences University and has a Masters Degree from Loyola University in Chicago.
He holds Diplomate credentials in sports medicine and nutrition, and is Board
eligible in rehabilitation, serving as editor-in-chief of "Spinal
Rehabilitation", a multidisciplinary reference text published by
Appleton-Lange/McGraw-Hill. He has hospital privileges at the Amery Regional
Medical Center and is certified in Manipulation under Anesthesia.
Dr.
Stude has authored many publications, some clinical case studies and some
original research manuscripts published in JMPT. He served for 16 years as
Associate Professor at Northwestern Health Sciences University, the first
in the profession to be budgeted full-time to help establish a doctor/mentor
model for training intern doctors in a clinical setting. He's had the
opportunity to serve in multiple venues professionally, some of which include
presentations at the University of Minnesota's medical school, coordinator of
post-graduate diplomate programs, and associate editor for
peer-reviewed publications.
Dr.
Stude has served as team physician for national and international sporting
events, some of which include the Ironman World Championships, World Cup
Speedskating Championships, World Powerlifting Championships, National Tae kwon
do Championships, Jr. Olympics, and Special Olympics. Dr. David Stude is
currently practicing in Eden Prairie, Minnesota at SouthWest Chiropractic.
